Plusieurs membres du réseau TeleConsult France (Philippe MEYER, Lionel PESQUER, Alain SILVESTRE, Benjamin DALLAUDIERE..) - Imagerie médicale de la Clinique du Sport de Mérignac - co-animeront avec les Cliniques Universitaires Saint-Luc de Bruxelles une journée de formation et d'échange sur le thème du Genou intitulée " les fondamentaux en moins de 20 leçons ".

La journée sera articulée autour de l'imagerie du genou et prendra la forme de courtes sessions (15-20 minutes) sur ce thème.

Le programme dédié à l’imagerie du genou traitera des fondamentaux, mais également des dernières innovations concernant cette articulation. Tout comme ils le font depuis plus de 2 ans pour le Centre Henri Becquerel de Rouen, les radiologues du réseau TeleConsult spécialisés en Oncologie (35 spécialistes à ce jour) participeront ainsi à distance à l'activité du Centre pour ses activités les plus propices, dans une logique de mutualisation des expertises et des moyens et dans le cadre du contrôle qualité organisé collectivement.

le Dr Marc Zins, Responsable du service de radiologie d'imagerie de l'Hôpital St-Joseph et président du comité Médical de Gouvernance et d’éthique de TeleCOnsult France animera Le Symposium Scanner Volumique, qui se tiendra les 25 et 26 janvier 2016 au Centre de Congrès Prouvé à Nancy.

Le Docteur Zins sera accompagné du Professeur Alain Blum, chef de service d’imagerie médiacal au CHRU de Nancy.

Au-delà de l’activité de tomodensitométrie, ce sont également des réflexions plus vastes qui seront abordés quant à l’organisation du workflow, des systèmes d’information et du temps radiologue à travers un segment qui sera intitulé : Scanner et PACS ou encore des temps d’échange autour de cas pratiques

Dans le cadre d’un nouvel appel d’offres, les Hôpitaux de Chartres (Eure-et-Loir) ont choisi de renouveler leur confiance au réseau TelConsult France pour l’interprétation de leurs radios et de leurs scanners en téléradiologie.

Les radiologues membres du réseau TeleConsult France mobilisés autour de la région Centre continueront ainsi d’apporter leur appui distant de diagnostic et d'expertise au service d’imagerie du CH.

Le Docteur Salah Bourbia, radiologue inscrit au tableau de l’Ordre des Alpes-de-Haute-Provence sous le N°04/1344 nous a rejoint.

Doctor Pierre Jaeger radiologist registered to the College of Physicians of the Loire county joined us. Former intern and ex head of Clinic within Toulouse university hospital, Dr. Jaeger relies on various skills including osteoarticular and ORL imaging.

Dans le bulletin Interne du CH de Chateauroux " regard sur l'hôpital édité début 2015 , les principaux intéressés dressent un 1er bilan de leur expérience nouvelle en téléradiologie ; un pari réussi :

Face aux difficultés rencontrées pour recruter un radiologue et afin d'exploiter pleinement son second scanner, le centre hospitalier confie, depuis juin 2014, à un partenaire extérieur, l'interprétation d'environ 20 % des examens. Avec à la clé pour le patient, des délais d'attente devenus très raisonnables.

Le recrutement d'un(e) radiologue est ouvert depuis 2012 mais face à la pénurie de médecins, l'hôpital de CHATEAUROUX s'est résolu à faire appel à un service « intérimaire », à des prestataires en télé-radiologie.
Après un appel d'offres et une rencontre avec les praticiens, la société « Téléconsult France » a été retenue pour optimiser l'exploitation des deux scanners (le second est opérationnel depuis septembre 2013) et assurer l'analyse d'une partie des examens réalisés en consultations externes.
L'objectif était de ramener les délais d'attente au niveau des normes nationales, soit environ deux semaines. Il est atteint : « Fin juin, il fallait attendre 60 jours pour un examen, un mois plus tard, ce délai était ramené à 30 jours.

Aujourd'hui, il est d'une semaine » constate Valérie DESCOUX, cadre supérieure de santé du service d'imagerie médicale.

Lors de la prise en charge, le patient est informé que l'examen sera réalisé en télé-radiologie, il doit consentir et signer un formulaire. « Cela demande de faire de la pédagogie, notamment auprès des personnes âgées qui, pour certaines, se méfient des nouvelles technologies ».

Les manipulateurs radios reçoivent les protocoles d'acquisitions par internet et envoient ensuite les images recueillies sur un portail sécurisé où les radiologues les récupèrent pour analyse.
Les Dr PRIVAT et REGNIER, de l'unité régionale de « Téléconsult France » basée à CLERMONT-FERRAND, sont les interlocuteurs privilégiés des praticiens et « manips radios » castelroussins. Les interprétations sont assurées par une équipe de 25 radiologues parmi lesquels toutes les spécialités organes sont représentées.

Les médecins du centre hospitalier peuvent également solliciter une télé-expertise si un résultat exige un second avis. « Le choix de cette société a été dictée par les excellentes références des radiologues associés, par leur démarche qualité et par leur souci de placer l'humain au centre de leur travail » détaille Martine GIRARD-BLANC-MOREL, médecin qui a participé à l'introduction de la télé-radiologie au sein du service. Un collège formé de professeurs d'université triés sur le volet sont en charge du contrôle qualité et répondent des meilleures pratiques de leurs confrères en terme de diagnostic.

Le contrat qui lie l'hôpital et « Téléconsult France » est très flexible : si demain le centre hospitalier de CHATEAUROUX recrute un radiologue, la collaboration pourra être interrompue sans délai.

The Digne hospital located in the Alpes-Maritimes area (south east of France) is now connected to the TeleConsult France network to ensure its readings of radios, Scanners and MRIs via teleradiology

It’s the third hospital to join us in this district. TCF’s radiologists based in the PACA region shall bring their remote support to relieve the imaging department.

The node of establishments in the region will most likely help sharing the experiences.

Le Docteur Julien FRANDON, radiologue inscrit auprès de l’Ordre des Médecins d’Isère nous rejoint. Il travaille actuellement en qualité d’assistant chef de clinique en imagerie cardiovasculaire diagnostique et interventionnelle dans la clinique universitaire de radiologie et d’imagerie médicale du CHU de Grenoble.

Le Dr Frandon justifie de compétences notamment en imagerie cardio-vasculaire et digestive mais également dans la prise en charge d’examens en urgences.

Il apportera ses compétences en priorité son soutien aux interprétations au sein de la région Rhône-Alpes.

Doctor Arnaud radiologist registered to the College of Physicians of the Hautes-Alpes and currently working alongside Dr Paul Philippe in the Manosque’s hospital, joined the network.

En marge des Journées Françaises de la Radiologie, le Pr François COTTON a organisé un EPU sur le TeleAVC
à l'attention des radiologues du réseau TeleConsult France.
Il s'agissait de faire le point sur les modalités d'imagerie, protocoles et interprétation
dans les AVC notamment à la phase précoce. La problématique de l'AVC reste une préoccupation majeure
des médecins radiologues lors de leurs astreintes (permanence des soins nuits, week-end et jours fériés)
pour les patients pris en charge dans les établissements rattachés au réseau (en IRM et au Scanner).

Ce premier EPU s'inscrit dans la démarche qualité organisée au sein du réseau et dans la volonté collective
d'une appropriation régulière des meilleures pratiques. D'autres sessions suivront...

The Chartres hospital located in the Eure-et-Loir area (28) is now connected to the TeleConsult France network to ensure its readings of radios. Scanners and MRIs via teleradiology

TCF’s radiologists based in this region and the ones next to it shall bring their remote support to relieve the imaging department.

This new hospital gives us an additional anchor in this region where the network already assist the hospital of Châteauroux since couple of months now.

Doctor Laurent Hauret radiologist registered to the College of Physicians of Gironde joined the network.

He will bring his support adding extra skills in Aquitaine region after the arrival within the network of 9 new members in this area.

Dr. Hauret relies on various skills including Digestive, Neurologic and ENT imaging.

We welcome him and wish him many successes in its involvement in teleradiology.

The Saint-Gaudens hospital located in the Haute-Garonne area (31) is now connected to the TeleConsult France network. TCF’s radiologists based in this region shall bring their remote support to relieve the imaging department.

We shall kick-off teleradiology activities in mid-October. By now, the service is accessible to around 40 users within the hospital’s walls in various departments (surgery, emergencies,...) that are stakey keholders in this project and which now can fully access the reports online.

This new hospital comes reinforce the existing cluster in this region in which the network already assists 4 hospitals : Villefranche de Rouergue, Saint-Affrique, Castres-Mazamet et Albi

The Saint-Junien hospital located in the Haute Vienne district, is now connected to the TeleConsult France network.

TeleConsult France’s radiologists will bring their remote support to relieve the imaging department run by Doctor Sylvie Taillefer.

We began teleradiology activities in mid-September. By now, the service is accessible to around 70 users within the hospital’s walls in various departments (surgery, emergencies, obstetric ...) which can now fully access the required reports.

Doctor Sylvie Mercier radiologist registered to the College of Physicians of the Rhône and working within the Saint-Louis Clinic, joined the network.
Dr. Mercier relies on various skills including osteoarticular and oncology imaging.

She shall bring his support to ongoing operations in the Rhône-Alpes region.

Doctor Laurent Rabeux radiologist registered to the College of Physicians of the North and already working part-time within the Quesnoy hospital, joined the network.

Dr. Rabeux relies on various skills including cardio-vascular, thoracic and uro-nephrology imaging.

He shall bring his support to ongoing operations in the North of France in priority for Arras hospital.

The Centre hospitalier du Forezwhich gathers Montbrison’s and Feurs’ hospitals (located in the Loire district) is now connected to the TeleConsult France network.

Many TeleConsult France’s radiologists mobilized within Rhône-Alpes will bring their remote support for both on-call and programmed activities.

These new establishments shall begin their teleradiology activities in mid-August for standard radio/scanner and later in September on their new MRI, this modality currently being installed on the Montbrison’s site.

As part of the tender issued by the hospital to find a partner able to support the radiology department, this establishment located in the Centre region has chosen to trust the TCF network for the remote interpretation of its radiological examinations.

It is more than 5000 exams that will be analyzed in the general frame of continuity of care which will take the form of an operational on-call availability during nights and weekends from our specialists and for day set vacations by organ’ specialty (radiography, scanner and MRI).

The hospital of Embrun joined us. Radiologists based in PACA’s region (southern France) shall provide support to the imaging department for the interpretation of conventional radios by organ’s specialty.

The Briançon Hospital joined TeleConsult France’s network.

Radiologists from TeleConsult France based in PACA (in priority) shall therefore bring their remote support in an operational on-call coverage and programmed activities by organ’s specialty (radio and scanner).

DoctorAlioune Badara FAYE, radiologist registered to the College of Physicians of the North and already working part-time with Doctor Arnaud Boulinguez within the Denain hospital, joined the network.

Dr. FAYE relies on various skills including vascular, gastrointestinal and oncological imaging.

We welcome him and wish him many successes in its involvement in teleradiology.

With the financial support of the Nord-Pas-de-Calais Regional Council, the Arras’ hospital was able to acquire a last generation MRI ready to operate at the end of April 2014.

To secure the operation of this equipment, and allow access to best practices, the Hospital has chosen to entrust part of its vacations to TeleConsult France’s network radiologists (local support in priority) who provide reports by organ’s specialty.

Both joined establishments forming the Centre Hospitalier Intercommunal of Evreux-Vernon (27) located in Northern France (Haute-Normandie) has joined the TeleConsult‘s network.

The support of our members in the medical imaging department directed by Doctor Slim Jouini first concern the cross-sectional imaging (scanner / MRI) by organ’s specialty for both institutions which shall begin in June.

As a reminder, TeleConsult’s members have been supporting, in the same region, the Henri Becquerel Center against cancer of Rouen for over a year now.

Sélestat's Hospital (67) located in Alsace has joined the TeleConsult France network.

The support of our members will mainly participate in the continuity of care for the establishment as an operational on-call duty support, starting by the end of April.

Doctors Patrick Baur and Alain Ott are already mobilized in the region.

The Grouped hospitals of Castres-Mazamet (81) located in the southern part of France is now connected to the TeleConsult France’s network. ( 37 000 ER visits per year).

Radiologists mobilized in Midi-Pyrénées will intervene in priority. TCF began to support the imaging department last weekend.

This is the 4th hospital in the region to seek our remote support by teleradiology.

The kick off for teleradiolgy activities have been planned for early April, but last weekend the Albi’s hospital was able to experience this organization in advance.

Thanks to the training provided and the preparation of the hospital teams upstream the previous week, the withdrawal of a replacing radiologist at the last minute did not prevent the radiology department to function normally.

So congratulations to all the actors in Albi’s hospital for their successful startup and to all the TCF radiologists who rallied to support the radiology service on such short notice.

The ESR (European Society of Radiology) in its ECR Newspaper article's entitled "French Society working to Ensure safety of teleradiology," highlights the work of standardization of the G4, whose SFR is part of,regarding Teleradiology and summarizes its charter in 11 points.

"At the national level, the role of the SFR is to promote healthcare policies which ensure patients have access to the best care and innovations throughout the country. Telemedi­cine, including teleradiology, is now part of healthcare, and the SFR sup­ports the development of new med­ical organisations for teleradiology, quality control and radiation expo­sure guidelines in order to improve patient care.
For teleradiology, the French Council of Radiology (G4), presided by the SFR and including all four representatives* of French radiologists, has noticed lower levels of patient care within teleradiology in some hospitals.
Such teleradiology programmes just do readings of CT and x-rays images with no validation of the requested examination, no support to the radiographers team, no management of the radiological department, questionable organisa­tion, and doubtful credentials from the remote radiologists.
With the French National Council of Physi­cians (Conseil National de l’Ordre des Médecins, CNOM) and the G4, the SFR promotes a teleradiology charter (see boxed text) which lists several key duties of the radiologist, along with ethics, and consideration for patients. This charter also refers to several other teleradiology guide­lines on medical convention, the role of radiographer, etc."

An article entitled "TeleConsult France: a network to improve the continuity of radiological workflow" was written on Thema Radiology website. This article briefly restates the principles of the structure while welcoming the arrival of another teleradiology organization into the network.

"The TeleConsult France project, which includes over 100 hospital and liberal radiologists structured by regions, recently expanded with the arrival of nine additional physicians. This is an opportunity here to expose the missions and values of such a network, which provides, since 2009, support several healthcare establishments via teleradiology especially regarding Permanence of care, scheduled activity as well as tele-expertise."

These radiologists have been involved since November 2011 in the remote support on continuity of care in addition to their hospital and liberal activities.

Over the past two years, they have participated in the remote interpretations for hospitals such as Villefranche-sur-Saône and Point-à-Pitre, but also in the context of regional teleradiology project in Lorraine (T-Lor) in several hospitals: Bar-le-Duc, Toul and Freyming Merlebach.

The proximity of the values ​​held by the two entities has facilitated this merger.

Doctor Hatem CHEIKH KHELIFA, radiologist registered to the National College of Physicians of Hérault ( Southern France), which works closely with Doctors Jean-François Vendrell and Olivier Alice at the Beau Soleil Clinic of Montpellier, has joined the network .

Dr CHEIKH KHELIFA has recognized medical imaging experience including musculoskeletal imaging, digestive and uro-nephrologic skills.

We welcome him and wish him many successes in his involvement regarding teleradiology.

Arras hospital (located in northern France) is now connected to TeleConsult France.

Radiologists within the network shall support the interpretations starting December.

The first partof the last issue of E =M6 broadcasted on November 17 untitled “Twins, how do they manage to synchronize their actions” in which Dr. Julien Savatovsky intervenes (Neuro-radiologist at Adolphe de Rothschild Foundation in Paris and also involved as organ specialist within the TeleConsult France network).

In this quarter’s issue of Technologies et Innovations Hospitalières, a french magazine orientated toward IT subjects in hospital environment ( p.64 No. 18 ) .

Radiologists working at the clinique du Parc : Olivier Fantino, Julien Borne et Bertrand Bordet, all of them committed toward the TeleConsult France network worked alongside System X (a French editor and integrator of solutions for medical imaging ) to develop M2V : a software designed to produce, transmit and save animated images. This is the first solution so far to allow a DICOM dynamic audio-video medical report.

"With M2V, the correspondent receives animated and guided medical images: a real revolution.There are many applications in endoscopy, ultrasound obstetrics scans, ophthalmology and particularly adapted for the preparation of a medical procedure. "

This tool allows to see, hear and understand real-time diagnosis issued by the radiologist, making it an educational tool evolved and therefore lays the foundation of tomorrow’s imaging reports.
